On October 1, 2018, the IRS released the 2018 1095/1094 final forms and instructions for IRS reporting under the Affordable Care Act (ACA). As expected based on earlier released draft forms, only minimal changes were made between 2017 and 2018 versions.
No change to Form 1094-B or Form 1094-C
In fact, aside from expected date changes, no alterations were made to Forms 1094-B and 1094-C when compared to the final 2017 versions. Links to the final 2018 IRS Forms 1094 can be found below.
Slight change to Form 1095-B and 1095-C
The IRS made minor changes to how names are entered on Form 1095-B and 1095-C.
Forms 1095-C and 1095-B previously provided only one combined box for first, middle initial, and last name. The 2018 final forms include separate boxes for each covered individual’s first name, middle initial and last name in Part III of Form 1095-C and Part IV of Form 1095-B (covered individuals)—and column (a) for both. The change was likely made because of the high number of TIN errors created by the current format. Links to the final 2018 IRS Forms 1095 can be found below.
